Let’s be real. The mental and physical health systems in the United States were built around a very narrow image of who a patient is. Cisgender, heterosexual, often white, often male is the standard of care. If you don't fit that image, you've probably felt it. In waiting rooms. In intake forms that don't have a box for you. In providers who smiled politely and still got it wrong, or worse, continued to misgender you or insist on treating you “the way it’s done.”
You are not the problem. The system is.

Getting support is harder when you’re queer or trans
Getting diagnosed with autism, ADHD, dyslexia, OCD, or any number of conditions isn't a level playing field for anyone who isn’t a white male. This disparity is especially wide for 2SLGBTQIA+ individuals.
Gender identity is pathologized
All too often, trans and gender-diverse folx enter an evaluator's office and find their identity treated as the problem to solve rather than a fundamental part of who they are. Being trans is not a symptom. Being queer is not a disorder. Any evaluation that can't hold that truth is neither safe nor accurate.
Diagnostic tools built on exclusionary research
The foundational studies on autism, ADHD, and learning differences used almost exclusively white, cisgender male subjects. The result? Diagnostic criteria that reflect a very specific presentation that routinely misses how these conditions look in womxn, nonbinary people, and gender-diverse individuals. If you spent years being told you made eye contact or were too social so you couldn’t be autistic, you’re too organized to have ADHD, or too smart to have a learning disability. This is why.
Masking is misread or missed
You probably became an expert masker at an early age, long before an evaluation even crossed your mind or your parents’. Years of navigating spaces that weren't safe, reading rooms and adapting to survive, mimicking “normal” people, can make it genuinely difficult for an untrained evaluator to see what's actually there. We’ve done our own unmasking work and it sucks, but it can be liberating with the right support. We see the real you.
Co-occurring conditions get collapsed into one narrative
Research consistently shows higher rates of autism, ADHD, and anxiety among 2SLGBTQIA+ individuals. Queerness obviously doesn’t cause these conditions, but because they frequently co-occur they often lead to a lifetime of stress, rejection, and the crushing weight of systemic injustice. Too often, provider discomfort or the pressure to take on greater patient loads means everything gets lumped under one label. Whether it’s capitalistic optimization or ignorance, it leaves you misunderstood and mistreated.
Barriers Don't Stop at the Diagnosis
Even if you’ve been fortunate enough to get the right diagnosis, the road to support is full of detours that straight and cisgender patients rarely have to navigate.

Provider bias: Studies have found that 2SLGBTQ+ patients are more likely to delay or avoid care due to fear of discrimination. From being misgendered in medical records to having a provider's discomfort visibly shift the conversation, this fear is reinforced in modern healthcare.
Heteronormative assumptions in therapy: When a therapist treats a client's identity as a variable to "work through," defaults to heterosexual relationship frameworks, or lacks any training in gender-affirming care, the therapeutic relationship becomes another place to perform rather than a place to heal.
Insurance, income, and access: 2SLGBTQIA+ individuals face higher rates of poverty, housing instability, and lack of insurance coverage than their cisgender, heterosexual peers. This is even more pronounced for BIPOC people and folx living in rural areas. When support is costly, access is hours away, and time off from work all factor in, care becomes a privilege, not a right.
Family rejection and isolation: For 2SLGTBQIA+ youth especially, family rejection remains one of the most significant barriers to care. Those who haven’t come out to their families, or who have been rejected by them, often can't access support through traditional routes. They need providers who understand how to navigate these realities and know that chosen family and outside support matter.
